Intake form

All clients are required to fill out a health history intake form prior to their first service.  This form must be updated once a year to maintain accurate records. Please fill out this form in advance to your appointment.

Physician's Permission form 

Doctor's clearance is required for anyone currently prescribed a blood thinner, or has had a procedure or surgery in the past 6 weeks, or if it is otherwise requested by the massage practitioner. This is for your health and safety. Please have your healthcare provider fill out this form (or write the equivalent information on a script paper) and return it to us by the start of your appointment.
